Here are the steps that you should follow :
Please read all the steps before you click on the button for payment.

1.Make payment to Pay Pal for IMC Billing Forms. This gives you only one week access to this site.

2.Fill out and Send Insurance Confirmation form by fax and overnight mail to your Insurance Carrier.

 The following documents must be mailed and faxed immediately to the address listed above :
  a) Assignment of Benefit Form
  b) Copy of your insurance card
  c) Right to Privacy Form
  d) Copy of your state drivers license
  e) Copy of the confirmation letter got from your insurance carrier in reply to the Insurance Confirmation Form sent earlier to them.
 

3. Make sure your carrier immediately sends the Confirmation Letter to IMC. Make sure this is done the day you send it in.  Call your Insurance Carrier every hour till this is done.

4.Call IMC and confirm if  the confirmation( Insurance carrier will send a confirmation to you as well as IMC ) has been reached. As soon as the confirmation has taken place you will be given a pass word. This pass word is good for one week. You must down load and save the billing forms during that week. If you fail to do this you will have to reorder the forms on Pay Pal again.

5.Submit
   a) Your operative report
   b) Your pathology report
   c) Your letter of necessity
   d) Pre authorization documents
    and all forms to the carrier.
   Do this by fax  and registered mail and call to confirm that these were received. When you  mail a copy make sure you staple the pages up the full side like a book. Insurance carriers often pull pages off and tell subscribes that pages are lost. If  you staple up the side this reduces the intentional destruction of your document.
   Call hourly to confirm your fax and daily on the OVERNIGHT EXPRESS MAIL to confirm when it is received.

6.Once you received confirmation of documents by carrier send the demand notice for payment daily via fax and call daily till payment is made to IMC for reimbursement of fees.

7.Your cases is on the case site for download for operative notes.

8.Read all the letters provided to you on insurance issues and print them now so that you have them for use later as needed.
